From San José to Guanacaste to the Pacific Coast — Costa Rica is one of the fastest-growing US veteran retirement destinations in the world. Here's exactly how your VA benefits and FMP coverage work — and what most veterans in Costa Rica don't know about their healthcare options.
Costa Rica is a fully covered FMP country. You can see any licensed Costa Rican provider for your service-connected conditions and submit for reimbursement. Costa Rica has a world-renowned healthcare system — the CAJA (Caja Costarricense de Seguro Social) public system is excellent, and the private sector in San José rivals US standards at a fraction of the cost.
Costa Rica advantage: Private medical care in Costa Rica is significantly cheaper than in the US — specialist consultations run $50–150 USD. FMP reimbursement covers service-connected conditions at any licensed provider. Costa Rica is also a major medical tourism destination, meaning hospitals and clinics are very experienced with international patients and insurance documentation including VA FMP claims.
Legal residents of Costa Rica can access the CAJA public healthcare system by contributing monthly premiums (based on income or pension amount). As a pensionado resident, your CAJA contributions are based on your pension income including VA disability compensation. CAJA covers general and specialist care for all conditions — FMP is then your additional layer specifically for service-connected conditions at any licensed provider. Having both is the optimal combination for veterans in Costa Rica.
Call +1 303-331-7590 or complete VA Form 10-7959f-1. Free. Register before you need care. Processing takes a few weeks. Essential for any veteran living in Costa Rica long-term.
The Pensionado visa requires minimum $1,000/month pension income — VA disability compensation counts. Once approved, CAJA enrollment is required. Monthly CAJA contributions give you full public healthcare access.
Costa Rican private clinics and hospitals produce clear Spanish invoices (facturas). Request itemized billing and medical reports. For FMP, an English summary or translation of key items helps but isn't always required. Submit to VA FMP Denver office.
Go to the nearest hospital emergency room (emergencias). CAJA public hospitals provide emergency care to all. Notify VA within 72 hours of emergency admission. Keep all documentation including hospital discharge report for FMP reimbursement.
Costa Rica's most internationally recognized private hospital in Escazú, San José. JCI accredited. Strong international patient services, English-speaking staff, experienced with US insurance and FMP documentation. Popular with US expats and veterans.
Central San José location. Long-established private hospital with English-speaking staff and strong international billing department. Good choice for veterans in the Central Valley area needing specialist care for service-connected conditions.
Quality private hospital in the northern Central Valley. Serves veterans in Heredia, Alajuela, and surrounding areas. International services available. Good option for veterans who prefer not to travel to San José.
Liberia is the primary city for Guanacaste veterans. Private clinics in Liberia, Tamarindo, and Playas del Coco serve the growing US veteran and expat community on the Pacific Coast. San José private hospitals are 3–4 hours for specialist care.
C&P exam availability in Costa Rica is improving as the US veteran population grows, but is more limited than in Asia or Europe. Exams are primarily conducted in San José. Veterans in Guanacaste or the Pacific Coast may need to travel to San José for their exam.
Give VA your exact Costa Rica address — call 1-800-827-1000. Without a Costa Rica address on file, VA will schedule you back in the US. Do this immediately when you move.
VA contractor exams primarily scheduled in the San José metropolitan area. If you're in Guanacaste, Monteverde, or the Caribbean coast, plan for a trip to San José for your C&P exam. Book accommodation in advance for multi-day visits.
Typically 2–4 months. Growing US veteran population means improving availability. Check with your VA regional office about current wait times and whether home exams or telehealth options are available for your condition.
Veterans in Costa Rica often have Costa Rican bank accounts, rental income, and FBAR obligations. The US does not have a comprehensive tax treaty with Costa Rica — making FEIE and FBAR compliance particularly important to get right. Monte offers FTARU forensic tax review for expat self-filers.
Monte works with veterans across Costa Rica — San José, Guanacaste, Monteverde, Caribbean coast, anywhere. Everything handled remotely via WhatsApp and secure document transfer. Costa Rica is in the same time zone as US Central — and only 2 hours behind Philippines time for most of the year.
Upload your VA C-File PDF via Proton Drive. Monte runs FNVI scoring — missed conditions, underrated ratings, nexus gaps — and delivers a written report with specific next steps.

No US-Costa Rica tax treaty means FEIE and FBAR compliance is especially important. Costa Rican bank accounts, rental income, and business interests create common filing pitfalls for US veterans. Monte offers FTARU forensic tax review specifically for these situations.
Monte reviews the C-File, identifies what VA used to deny or underrate, and builds the supplemental claim or appeal evidence package remotely from anywhere in Costa Rica.
